Pathology of Pancreas and Islands of Langerhans in Diabetes type 2. March 22, 2017

       Books I ordered just a few days ago no longer on market in Amazon. I do understand, they come-in and go -out. Still, when they go out it is difficult to say when they will come back. These books out-of-print, back from the post Century. I am interesting in all books about diabetes published before 1980. I am not sure that it was the time when Diabetes type 2 took its roots. But before 1980 as I suspect every diabetic old of child, fat or skinny were treated with insulin. Then insulin was replaced by SU for adult diabetics, and not these medicine already started to replace diabetes treatment for children. 
      It is easy to understand that if today child diagnosed with diabetes type 2 then tomorrow child would be diabetic type 2, in no one way child would be diabetic type 1.  Many times and very long I try to find out, how type of diabetes diagnosed, based on what? And there is no one answer to so simple question: why I am diagnosed as type 2 diabetic and not type 1? 
      Right now for me really it is no longer the point of interest. I do have insulin delivered to my door every three month, so regardless I am type 2, type 1, or LADA or any other name it will be given, I do have treatment I need, insulin, Lantus solo Star. 
      Still, I try to educate myself. So I ordered all books which are not one after another go out of market, old published books, and I read them. I proud, I own 1928, 1934, and so so on. I started to order all medical library, diabetes, heart, lungs, urology, and so so on. These books are medical test books, but students no longer order them, They have to buy new versions, a new editions. I am interesting not in the newly developments of treatment but in the process of development of treatment. 
     The picture with old test books and old published books is very interesting. In past there were more valid info then it is right now. Right now I read book just a few days, and then I put it aside, no any value in it. Books in past, back to 1930 or 1919 are very informative. Of cause with time there would be re-prit these books, And I really do not trust in re-print. It is like to listen to the opera in Opera House, or recording. It is great to listen to the opera in any place and at any time, still, best place is Opera House, original singing.

  • Book published back to 1930. It is fantastic! This book give answer to the questions I do have today, almost 100 years after it was published. No one contemporary medical test book have so valid info like this very tiny old book, just little bit more then 200 pages. 
            Author,  Warren, Shields, give full picture pathology of pancreas. Of cause, if today book like that would be published then it would have more different cases of pancreas pathology. There are no books like that.
          Diabetes, nether type of it, in modern medical education presented and considered as Metabolic Disorder. If so, then there is no one reason to pay attention to the pathology of Pancreas or Islands of Langerhans.  All what diabetic need just right meal plan. Back in 1930 meal plan also was important part of treatment diabetes mellitus. But it was insulin which was injected by every diagnosed diabetic, old or child. 
         Now back to 1930 and  Warren, Shields where he describes different cases of Pancreas Pathology. 
    Hyaline change. Hyalinization of the Islands of Langerhans - degeneration of Island cells. Common in young diabetics and in old.  In most cases, and it is 1930, the pathology in diabetics aver 40 years old.Also it was said it was now presented in non diabetics but remember it is 1930, and the level of sugar was tested mostly in urine. So, probably it was diabetic but not diagnosed with diabetes.
    When the process become sufficiently marked at destroy a number of the epithelial cells or to separate them from the blood stream by a practically impermeable membrane, diabetes will developed. (page 42)
    This is diabetes type 2, diabetic over 40 years old. No one do know when diabetes was developed. Only autopsy show the pathology of pancreas. Why today, almost 100 years after insulin already discovered, the disease of Pancreas and Islands of Langerhans considered as it is Metabolic disorder? Doctors in medical clinics with full confidence measure my middle area and present it is resulted diabetes type 2. What they never look at that diabetics type 1, children, just bones, back to the beginning of Banting Era, have wide middle area. Really, just take a look at the pictures of those kids, just bones and skin, and ... middle area is wider consider with non diabetics.  They do have belly. The same as Asian today. They meet criteria of BMI but they do have prevalence of diabetes.a then those Asians who are not diabetics.
           There are other pathology described in book. Some pathology only in children, some pathology only developed with age, some progress speedy, and other progress slow. In all those cases most important that Insulin would help to every one, and in some cases with insulin treatment pancreas healed, Islands of Langerhans started re-generation. This process never possible with SU and other oral medicine. This is why today those of us who was diagnosed as child with diabetes type 1 live almost regular life. Other, myself including, diagnosed at 50th, when process of degeneration took its aggressive form. If one is lucky, as myself, and switch from SU to Insulin at time, then one can survive. Otherwise, less then 10 years and diabetics type 2 die, usually stroke, heart attacks, and COPD when breathing simple stopped.
