Medicare approved CGM for people 65+. Anne Peters, MD, very happy to educate Americans and all who wish that now diabetics over 65 can be reimbursed by Medicare to use this system. What I really do not understand, what is good to use it? Yes, it is first look that to know level of sugar in your blood at any moment just a dream for diabetics. There is question, what to do with this level of sugar in blood when it is detected as high, or low, or normal, any level it can be, how to use this info?
For those who are not diabetic it sound stupid. Really, what do I talking about? It is good to know numbers, and we all check that numbers many times daily. Right? Right. So what?
There are many way to address to diabetic's needs, but first one is all the time, how to get that numbers under control. Does CGM system will lead to better control of numbers on meter? Not at all. It is just info, and after that actions must be taken. What they are, actions how to control blood sugar? How to treat diabetes, the endocrine system disorder? In all papers, in every education classes, in text books and ADA publications diabetes presented that it is elevated level of sugar in blood. All time all treatment addressed to the action to lower blood sugar level, or to rise it if it is already too low. But diabetes is not level of sugar in blood. It is disease of pancreas which does not secret insulin according to body system demand. SO, how CGM or any other devices to monitor blood sugar level effect the endocrine system?
Usually it is easy to say that if we do know what is the level of sugar in blood then we can adjust dose of insulin, and this action will lead to control blood sugar level. True? False. There is no one table how to adjust dose of insulin according to the real time blood sugar reading.Anne Peters, MD, Suppose to know it.
Another point is, the system approved for diabetics 65+. According to the author of publication diabetics were on that system and suddenly after 65 they must give up it, to whom? CGM is individual system. It cannot be used by any one else after it was delivered to the patient. Even patient never used it it will be considered used. It cannot be returned and delivered to another diabetic. So, why it is so sadden that diabetics who used the system, do not have it anymore? They still have it. They still use it. There is another interesting point, why today CGM will be used by diabetics type 2? That's right. Diabetics type 1 already do have CGM before age 65. They are type 1 diabetics.People 65+ and diabetics type 2, who take oral medicine such as Invokana, Metformin, Glipizide (SU), and all other trash medicine. For them CGM never was available. SO, what is now? Why suddenly Medicare started to accept to pay for system? Those type 1 already do have Medicaid, so they do not need that Medicare pay for system. They do have both, medicare and Medicaid, so they do have CGM if they need it. Type 2 diabetics have nothing. And it is 95% of diabetics population.
As usual, follow money. And remember, once system delivered manufacture will get money, does not matter if diabetic will use it or not. Usually we take it. If I do not take it then I will lost many other medical devices or medicine I desperately need. So, I will take it. Would I use it? No. Just think about. This device all the time attached to your body. Infection on the side. Infections on the side of insulin injections. Many other types of infections we are easy to catch. Do I really need this device? Not at all. I am OK with finger pricks.
BTW, system cannot be trusted. Two times daily we have to calibrate it, to use old fashioned glucose meter. And strips. So, what is the point, do I really fancy to use this CGM?
