"The initial dose of insulin should not be more than five units twice a day. This amount can be gradually increased until the patient becomes sugar-free, the diet being kept constant"- Dr. F.G BantingThis was written in 1924, almost 100 years ago. Today the quality of insulin is different, and so the regime of insulin therapy can be modified. Still, the first initial dose suggested by Insulin Developer Dr. Banting remained the same. It is better to start with 5 units two times daily, or 10 units one shot a day. After the dose of insulin should go up till the target of blood sugar reached. In the time of Dr. Banting the diagnose of diabetes was not based on level of sugar in blood but in urine, which is lower then in blood.
First time till I was diagnosed with diabetes and started to look at closer to the professional text books and other medical materials, Insulin Talks started appears in medical publications. When I read the discussions and suggestions how to Rx insulin for diabetics type 2, I see, besides of almost 100 years of being on market for medical pro Insulin is Stranger. They simple know too little about most powerful medicine which ever was created. There is one out of ten Americans is diabetic. Every primary doctor has many patients who are diabetics. In every hospital on every floor at any time there are some diabetics, and not too many professionals who really know how to reduce level of sugar in victim's blood.
It is sad and shame. Thousands diabetics die every year due to high blood sugar level, 100% preventable Death with proper insulin type and insulin regime therapy. There are no doctors who able to handle the problem, to treat diabetics otherwise then with bathroom scale and Barking Therapy. We should not die today because of diabetes. Regardless what we should or not, diabetes is number 7 cause of Death in America. Why is this so? Because of treatment which recommended by WHO or ADA does not work. It is not only does not work, but in contrary to heal the treatment kills and destroy other diabetic's already effected by diabetes organs.
Right now still it is presented that in early stage of diabetes or pre-diabetes when sugar is abnormal but still lower then threshold to be diagnosed as it is diabetes insulin should not be Rx. The question is, why to wait? What we suppose to wait? When diabetes will take full power and there would not be time to escape? I still not able to get this medical logic, why it is not standard of medical care to catch disease early and treat it with less complications? Why wait when insulin only would be able to stop the progress to the grave? Still, I do not get the point.
In all publications it is low blood sugar which taken into account that with early insulin therapy it will lead to low blood sugar. It will not. Then blood sugar is higher then lower it drops with insulin injections. Only when my blood sugar was above 400 and ++++ my lowest level dropped below 40 mg/dl. With insulin highest level started to drop, so lowest level started to rise. Now I can feel low blood sugar, so I will take actions before it drops too low, but it is level of sugar 60 mg/dl or 70 mg/dl, and sometimes I feel low even with 80 mg/dl.
As Dr. Banting stated we can feel not really sugar level low but the speed of blood sugar dropping. This is why with perfect level of sugar I can feel it is low. It is low blood sugar awareness, so there is nothing to worry about, just be ready to take action. In no one way this level of sugar will drop too low, dangerously low, fatally low. It will not. Just one protein bar, and problem solved. I never take candy or some sweets. I prefer to take OJ, or protein bar, or better to eat soup, hot soup.
In contrary, if highest level of sugar is way too high, above 400 and so on, some people admitted to hospital with 700 mg/dl and even 900 mg/dl, then they placed under IV medicine dropped into vein, and .... blood sugar drops below surviving level. Diabetics die in hospital because of low blood sugar.
I was in dangerously high blood sugar level when my blood sugar run above meter limit, 599 mg/dl. Insulin saved my life. I did not take high dose of insulin at once when sugar is too high, no way it is good, it is danger. I take it slow, one shot after another, smaller dose, more shots. I am very well tolerate high dose of insulin. Right now I injected 120 units, because of I skipped one shot at day time, simple forgot to take it. But if blood sugar is high I will not take it high dose, less then 80 units, and even 60 units or 40 if sugar is too high. I can take another shot one or two hours after, then another shot two hours after, till sugar start to go down.
As any medicine the same insulin, takes time to start to do the job. For this reason there are different types of insulin, rapid acting insulin which start to work withing from 5 to 20 minutes after injection, slow release insulin which does not need to take with food. In hospital in case of high blood sugar it is rapid acting insulin which would be injected to decrease level of sugar as fast as it can be done. The problem with this is, that after one or two hours blood sugar run up. At least this regime does not work for me. I do not try to decrease blood sugar fast. Of cause when my muscles are twisted and I am in fear to get heart attack that my heart muscles would not be able to relax, I will take rapid acting insulin, Novolog. 10 units, or even 25 units. I need high dose of insulin and as I said I well tolerate any dose of insulin. But I will not take rapid acting insulin if I am not in dangerously high level of sugar. When I do have insulin in my ref I never run into high blood sugar. Sometimes it can be higher then 300 mg/dl, but all the time less then 400 mg/dl. So do low level. It can be 56 mg/dl, or even 52 mg/dl, and even 40 mg/dl still not fatal.
