Type 2 Diabetes: When Does It Start? July 12, 2018

Conclusions FPG was significantly elevated in those who developed diabetes at least 10 years before diagnosis of diabetes, and this was also the case in those who developed PDM. Glucose dysregulation precedes diagnosis of diabetes at least for 20 years.

 OK, diabetes type 2 started about 20 years before it was diagnosed. After being diagnosed with diabetes type 2 diabetics gone less then within 10 years. What is conclusion?
    Japanese people are relatively lean, and the average body mass index (BMI) in the general population is ~22 kg/m2, whereas that of patients with diabetes in a representative cohort was slightly >23 kg/m2. Increase in insulin resistance before clinical diagnosis of diabetes was reported to be modest, irrespective of participants' body weight in the Japanese population. Despite the low degree of obesity, accompanied with increased Si in the Japanese population, the trajectory of glucose before diabetes and the prevalence of diabetes are not substantially different between Japanese and whites. 
It is well known that Asian population has higher diabetes type 2 prevalence then general American population. The conclusion? Body weight is not lead to diabetes type 2 development. 
Despite accumulation of data, the timeline of diabetes has not been fully understood. Particularly, the onset of glucose dysregulation leading to diabetes has not been clarified, i.e., the trajectories before diagnosis of diabetes have been assessed for 10 to 15 years, and the patients destined to develop diabetes exhibited significant elevation of PG already at the earliest time points in all studies.
So, with all that studies and publications there still no data what tests show that diabetes started. Sounds weared, but think about, what test shot, person gas diabetes type 2, if disease started to show up 20 years before standard tests measures? For 20 years every one entered in clinics not once, and still, no one test for diabetes. It is shame, is not it? Test cost so little. There is no time to shared  by doctor. It is simple drop of blood out of finger, or A1c test when blood work done. Why it still level of sugar not being vital? Probably, if this will be done then it will be clear that body weight or shape is not the leading cause of diabetes type 2. And if so, then what MD suppose to do? What MD will Rx to treat diabetes on the early stage of it? To lose more weight? To stop to eat  once and forever?
Diabetes was diagnosed when FPG ≥ 126 mg/dL or HbA1c ≥ 6.5%. Diagnosis of diabetes was also made based on information by the participants who had been diagnosed with diabetes at a medical facility. PDM was diagnosed when FPG was between 100 and 125 mg/dL and/or HbA1c, 5.7% to 6.4%. Participants with FPG < 100 mg/dL and HbA1c < 5.7% were diagnosed as NGR.
It is good to know that all levels of sugar under FPG <126 mg is normal. Or HbA1c>6.5%. Now there is the question, is level A1c and FPG are the same, interchangeable?  It cannot be. Why? A1c is the level of sugar average  from 2 to 3 month. So, it is include fasting level of sugar, and elevated level of sugar after meal. In contrast, FPG is only one condition, lowest level of sugar everyday. It is different every day for those who are diabetics. And it is practically the same for those who are not diabetics.
  • An A1C of 5.7% – 6.4%
  • Fasting blood glucose of 100 – 125 mg/dl
  • An OGTT 2 hour blood glucose of 140 mg/dl – 199 mg/dl

 It is another tricky point. If normal level of sugar is 140 mg/dl then why diagnose of diabetes type 2 only if level of sugar over 200 mg/dl? What are those tricks that it is diabetes, pre-diabetes, or non-diabetes, or insulin resistance? If the temperature of body must be 36.6C then elevated body temperature is already started to be treated. We still try to use remedias, but we have medical condition, and we try to treat it.
It is not the same with diabetes type 2. Regardless it is 100 mg/dl, or 200 mg/dl, or 800 mg/dl, the treatment is the same, lose weight, modify your life, eat less, work more.
       Take a moment to look at these numbers. These numbers when diabetes, probably, would be diagnosed. And before this happy day will come 20 years diabetics type 2 "developed" diabetes, progressing from 100 mg/dl to 700mg/dl. And pay attention to this fact, it is still the same diagnose, type 2 diabetes, insulin resistance. These diabetics type 2 still not have therapy they need, insulin to safe the lives. They must "reverse diabetes type 2" with diet and work outs. Try it. I did. I got stroke. But still there is no diagnose that I did have strokes, and still no medical attention to it. Simple. They need 20 years to see how medical condition progress to fatal end.
